Disclaimer

This project has two objectives:
  1. To highlight interventions cited as best or promising practices in the OPHS Guidance Documents
  2. To provide current, complete and consistently formatted (using THCU's 6-step model) examples of issue-specific program plans

Some of the interventions were created a number of years ago. We have done our best to add information to the plans to make them current, however, even more current information may be available. These plans are a starting point only.

In some cases, not all of THCU's 6-step planning processes were completed, or records were not kept. In order to offer complete plans, we have added sample fictional text in some parts of the plans.

Occasionally, the real community-level plans on which these sample plans are based were not done in a way that matches THCU's practices (for example, formulation of outcome objectives). In these cases, we have modified, replaced, or added new information on top of existing documentation.
