For vaccine eligibility criteria and booking information visit the Government of Ontario's website or call the Provincial Vaccine Information Line at 1-888-999-6488 (TTY 1-866-797-0007).
For a copy of your vaccine receipt visit COVID19.ontariohealth.ca. If you were vaccinated in another country/province and have questions about updating your health records, please contact your local public health unit for more information.
