Electronic Water Test Requisition

Submitting your sample information is as easy as 1, 2, 3, 4.

  1. Simply complete the requisition below by filling out all required fields (*).  Only one form is required per water sample and you do not need to send a paper form for this submission.
  2. Be sure to keep your barcode from the water collection bottle as you will need this information to access your online report.
  3. Drop off your sample at a drop-off location as soon as possible (within 48 hours of collection. Be sure to collect your sample properly as the accuracy of your test results depends on it.
  4. Get your results online in 2 to 4 business days through the “Get Results” tab on this page.

** Remember to keep a barcode from the water collection bottle and the last name you entered; you'll need them to access your online report.
Your results will be available online via the Water Testing Portal within 2 to 4 business days after your sample is submitted.

Drop off your sample as soon as possible after submitting the electronic form. The sample must be tested within 48 hours of collection or you will need to take a new sample and submit a new form.

Terms and Conditions

  1. The sample was collected from a “Single household”: Water intended to be potable and for human consumption collected from a drinking water system (e.g., a well) that serves one or fewer than six (6) private residences. It is not from a regulated drinking water system (i.e., water for public consumption, e.g., city water) or from a commercial bottled water product.
  2. By submitting the water sample for testing, the submitter represents and warrants that the water sample was taken from the Location of Water Source indicated in the test requisition and that the information provided is true in all material respects at the time of submission. The Ontario Agency for Health Protection and Promotion assumes no responsibility for the accuracy of the information provided, the manner in which the sample was collected or the mode by which it was transported to the laboratory.
  3. The submission must meet the acceptance criteria or the sample will not be tested.  A new sample and associated form must be submitted.
