Case and Contact Management

In Ontario, Diseases of Public Health Significance (DOPHS) must be reported to public health to support disease management and surveillance. Timely reporting into the integrated Public Health Information System (iPHIS) supports surveillance and assessment of disease mitigation strategies. PHO provides scientific and technical support to public health units investigating DOPHS cases, contacts and outbreaks.

Authorized public health unit professionals can access resources for DOPHS reporting and surveillance through the iPHIS and Cognos Document Repository (the repository), including iPHIS resources including user guides, bulletins, best practices, as well as Ontario Investigation Tools (standardized questionnaires) for case investigations.

Access to the repository is restricted to authorized public health professionals

To request access, email Communicable.DiseaseControl@oahpp.ca with the subject line: iPHIS and Cognos Document Repository Access Request - (Your PHU or organization's name).
