Youth Wellness Hubs Ontario (YWHO) serves as Ontario’s Integrated Youth Services (IYS) initiative, delivering integrated mental health, substance use, primary care, and social supports for young people aged 12-25 and their families. Functioning as an evidence-generating learning health system, YWHO actively uses an integrated data platform - My Wellness Passport - to implement measurement-based care (MBC) across its provincial network. This PHO Rounds will provide a comprehensive overview of YWHO, describe the co-design of this model with youth (including Indigenous youth), and share learnings on the implementation of MBC including a current data snapshot. The session will share experiences and lessons learned on how the YWHO model is being used and adapted to support Indigenous youth and their families.

Intended audience: Medical Officers of Health (MOHs)/Associate Medical Officers of Health (AMOHs), public health professionals with interest in child/adolescent health and health promotion, public health program decision makers.

By the end of this session, participants will be able to:

• Identify how to access to Integrated Youth Services (IYS) in Ontario.
• Describe co-creation frameworks which can be used to design service models alongside youth, with a specific focus on Indigenous Youth engagement.
• Apply public health prevention strategies into their work with youth.
• Discuss how to implement an integrated model of care for mental health, substance use and community supports service.

Marla Banning is the Integration and Implementation Lead for Youth Wellness Hubs Ontario (YWHO) at the Centre for Addiction and Mental Health (CAMH). She has been with CAMH for 26 years, including the past seven years with the YWHO Provincial Office. Throughout her career, Marla has supported numerous implementation projects, often focusing on youth mental health and justice-related initiatives. She feels privileged to have collaborated with more than 15 communities across Ontario to implement the YWHO model, helping improve access to integrated health and wellness services for young people.

Jonathan Berges is the Integration and Implementation Lead, Youth Wellness Hubs Ontario (YWHO), at the Centre for Addiction and Mental Health (CAMH). He has been with the YWHO Provincial Office since 2017 and has helped support the growth of the YWHO network from four hub networks to 32 networks serving communities across Ontario. Drawing on more than 20 years of experience in the youth mental health and developmental services sector, Jonathan brings a humble, community-first approach to his work. He is committed to building strong, sustainable partnerships that build on community strengths and support positive outcomes for young people.
